Why Transformations Fail When They Should Succeed.

I have spent 15 years in patient access, reimbursement operations, and change management — inside the specialty pharmacy programs where healthcare's friction actually lands on patients.

Since 2018 I have watched the same problem wear different clothes. I drew a specialty pharmacy HUB architecture with a routing-engine slot marked "T-B-D," because the AI to fill it did not exist yet. The AI exists now. The box is full. And the organizations around it still are not ready — because the constraint was never the technology. The technology moved. The people dynamics did not.

That is why I built ARC, the AI Readiness Continuum. AI is not additive like the cloud or the EHR before it. It is substitutive — it absorbs judgment, and it displaces the authority and identity of the people who held that judgment. You cannot ADKAR someone out of grief — no change-management playbook covers loss. A framework for this era has to measure data, people, and governance together, and it has to keep the patient's path as the metric that matters.

The work behind the practice

Proof, Not Positioning.

On a federally mandated overhaul of the pharmacy claims standard, we built the traced knowledge system a national program was missing: a 600-entry knowledge base in which every requirement cites its source, and every source is pinned to its page in the standard. Reconciling the program's working inventory against the published standard recovered 57 in-scope changes that had silently dropped off the list — gaps that would otherwise have surfaced as defects after launch.

In an AI readiness engagement for a financial-services advisory firm, every rating traced to an evidence register with graded confidence, every interview question had to show which judgment it informed or it was cut, and the findings were argued against before the client ever saw them. The verdict — ready for controlled adoption, not for AI search across its own files — saved the leadership team from funding the wrong first project.

That is the standard the practice runs on: assessments that show their evidence, requirements that cite their sources, and AI that is governed before it is trusted. The same discipline now ships as the ARC instruments — Readiness, Governance, and Adoption.
